In vitro study | Fosbretabulin disodium (Combretastatin A- 4 phosphate disodium, CA4P disodium) is A water-soluble prodrug of Combretastatin A4 (CA4), which targets microtubule and was originally isolated from the African tree Combretum caffrum. CA4 is a tubulin binding agent, which binds to or is close to the colchicine binding site of β-tubulin (K d = 0.40 μm) and inhibits tubulin assembly with IC50 of 2.4 μm. CA4 is cytotoxic to proliferating but not dormant endothelial cells and has potent, selective toxicity to tumor vessels. CA4P(1 mM,30 min) disrupts the endothelial cell microtubule cytoskeleton and mediates morphological changes in endothelial cells. CA4P stimulates actin stress fiber formation and membrane bleb, and increases monolayer permeability through Rho/ Rho kinase. CA4P increases endothelial cell permeability, mainly through the stem VE-cadherin/β-catenin/Akt signaling pathway, and inhibits endothelial cell migration and capillary formation, resulting in rapid vascular collapse and tumor necrosis. |
In vivo study | CA4P administered alone at 10% of the maximum tolerated dose (MTD) in an experimental tumor model resulted in rapid, extensive, irreversible vascular closure. After CA4P treatment for 6 hours, the vascular volume decreased by 93%. CA4P at a dose of 100 mg/kg for 6 hours decreased about 100-fold in tumor blood and about 7-fold in spleen. |
